We have a user who left their office and when they came back their phone was forwarded to a number they weren't familiar with. They're asking whether we have any ability to check WHEN this forwarding was set up. I'm not sure where to check. Ideas?

No way to look into that, besides CUCM traces.

Hi Wayne,
I've had the same problem as well with one user. She was a personal assistant using a 7962 phone. when ever the IPMA manager's phone status update would come up, the user would press exit to return to the normal phone buttons and accidently press the cfdwall softkey without realizing and then punching in a number.
what I ended up doing was re-arranging the softkey buttons and I put cfwdall by the second display so that one would have to press more to get to it.
Perhaps you can check your softkeys as well to verify how they are arranged.
