What is the best place (gateway or CUCM) to perform the translations for called and calling (inbound/outbound) numbers when I have Call Manager 7 with a ISR?.

Well... it depends.
If you like MGCP, then on the CallManager, as you don't have any choice. This also means any dial plan stuff you have is configured in one place (CCMAdmin) so is theoretically simpler.
If you like ultimate flexibility, then H.323 opens up a lot more options... but is more complex initially (and once you've started getting clever with it).

Sometimes you just need to block some calling numbers, it's better to do on ISR.
