Hi
As the title says...
Two people in a call. One on PSTN, one on IP Phone (UCCX Agent). Person on PSTN hangs up, guy on the IP Phone gets busy.
Any ideas on what could be the reason?
This is a problem for us as the Agent needs to disconnect the call manually and set himself to "ready" again. As before, when the other party hung up, the call was disconnected immediately and the Agent became ready again.

What type of PSTN connectivity, what signaling protocol between the gateway and CUCM, and what versions of IOS and CUCM?
The first step is to see what the PSTN sends when the far-end disconnects. If you're using an ISDN PRI turn on debug isdn q931 and include the output in your reply. If you're using SIP then debug ccsip messages.

I noticed you said, "As before" indicating is used to work. What has changed?

Indeed, it used to work just fine. I do recall installing a new device "pack" in order to be able to install ATA 187 's.
Could that have triggered our issue?
