i have this bug on my system, one device with 2 extentions modules 7915 resetting multiple time by day.
The CUCM version is 9.1.2.14091-1
The version of 7962 ip phone is SCCP42.9-4-2SR1-1S
the version of Extention module is B015-1-0-4-2

The phone firmware is what needs to be applied but that fix is only for 7962 and 7975 models not 7915 and as Mohammad stated you will need tac to provide it, its not under general release

Is this issue resolved. I just tried to downgraded phone firmware and monitoring it.

Upgrade to 9.4(2)ES11 or higher firmware version.

I have the same issue running CUCM version 11.0, you need to upgrade the phone firmware

Hi,
The bug fix looks to be ES firmware and needs to open a TAC case in order to get it. I don't think you will find it listed is cisco downloads
