Is there a way to determine which call manager (not its IP address but whether primary, 2ndary, teriary) a Cisco IP phone is registered to in a SIP register keepalive message?

No. Either go to the phone's HTTP server and see the IP address/hostnames in the list; or, look at the CallManager Group membership in CUCM Admin.
