incomplete voicemail with Unity Connection 7

We got a partially recorded voicemail with CUC. Please check the attached .wav file. What is the possible issue?

Hi,

There could be a lot different reasons for this.  Can you provide a little more information?

Has this happened before?

Did it happen to more then one subscriber?

What does the call flow look like?

I believe it is the first time seeing this problem. I am collecting more information from the customer.
