IP Phone shows systems in fail over but cluster is up and running with no issue's. We intermittanly have one phone that will show system fail over and hold is grayed out while off hook. We don't have option 150 configured on the switches but we do have a DHCP server that has that option in it. So i'm wondering because we don't have it configure on the switch but use a TFTP server could this be the problem?

If phones go into SRST mode is because they lost communication with the CUCM servers. And that can be for any number of reasons.
No, phones either have their configuration manually, which disables the DHCP requests, or use DHCP to get it, they don't mix.

Is that phone that is in SRST mode in a separate subnet from all the phones that are working? can you actually ping the cucm's from the subnet the phones are in. If you take a working phone, unplug it and replce it with the misfunctioning phone, does it start working then? if so, definitely connectivity.
Focus your troubleshooting on connectivity issues.
