Hi Leonardo,
As per my understanding the ActiveLoadID would be the firmware that is actually running on the IP phone , while the LoadID is the one defined on Device defaults or Phone Load Name setting on the phone config page. You can verify the same by analyzing LoadID and ActiveLoadID for one of the phones where they are not matching.

Does anyone know for sure that if a phone has a load specified at the device level (different than the device defaults) but the running code matches the device specified code, does it get flagged as a 'mis-matched load' ?

Hi,
On phone page you should find active and inactive load if the phone firmware has been recently upgraded with the current available firmware on CUCM.
If you find active load only it means that the phone matches the leatest available firmware on CUCM or it has never been aligned due to a communication error with the TFTP
