We have a group of 7942 phones that are not logging missed calls. Not all phones are affected.

+5 to Islam for pointing to good link, as most of such problem occurs just because of firmware misbehave/bug.
Apart from of it, make sure that you have checked "log missed calls" on line setting page of phone.
