Softkey Labels get "Temp Fail" when connected to a call. Cisco 7962

Current situation: The Phone is @ reception Desk. Whenever a call comes through, there is a message appear "Temp Fail" when connected. which make the reception don't have options like, to Transfer/Hold/etc. 

these do not happen all the times, sometimes it appears just fine and the user can interact. 

 

Below steps already tried :

1) Change the Softkey Template/Phone Button Template.

2) Change Cable.

 

so can anybody help me? please
