The first question is whether the exact versions you are running are listed in the CCX compatibility matrix? There have been problems where a change in AXL or CTI Manager (of which there were many in CUCM 8.6) wasn't expected by CCX and broke things.
Second, use Route Plan Report to check if any of the extensions already existed in CUCM. For example, if the CMG existed before and you deleted it there may still be Directory Numbers with those extensions in CUCM. CCX will not overwrites these since it has no way of knowing they aren't used for something else. Delete them from CUCM manually; same goes for CTI Ports if the name is the same as what you're trying to create.

In addition to good tips from Jonathan (+5) review the MIVR logs on UCCX for any clues on what is causing it.
