I am setting up a vic2-4FXO/DID in a 3845 router as a personnal experiance as below.
IOS is c3845-adventerprisek9-mz.151-4.M3
voice-port 0/1/0
signal loop-start
cptone us
ring frequency 25
no shutdown
voice-port 0/1/1
signal loop-start
cptone us
ring frequency 25
no shutdown
dial-peer voice 401 pots
desttination-pattern +14085553737
port 0/1/0
dial-peer voice 402 pots
desttination-pattern +14085554141
port 0/1/1
num-exp 3737 +1408555373
num-exp 4141 +14085554141
Scenario consists of 2 handsets which have to call each other by 3737 or 4141
everything seems to be fine.
except, when I dial the other number, i get a busy tone & 2ndd handset doesn't ring.

Why do you want to plug handset(analog phones) in FXO ports ?

I am using FXS module.

One of your num-exp seams to be wrong, num-exp 3737 +1408555373
Change this to num-exp 3737 +14085553737

It was a mistyping. The router configuration is correct.
