Could you let us know the exact versions of CUCM and CUE that you are running?
Also from CUE the following outputs would be useful:
> show call-agent
> show ccn stat ccm
> show ccn sub j
Is there a firewall between Unity Express and CUCM?
Thanks,

Hi Syed,
According to the CUE design guide having a firewall between CUE and CUCM is not supported:
"Java Telephony Application Programming Interface (JTAPI) communications does not currently work via NAT or firewall, so you cannot configue Cisco Unity Express on the other side of NAT from a Cisco CallManager controlling calls via JTAPI."
http://www.cisco.com/en/US/docs/voice_ip_comm/unity_exp/design/design21/cuenwinf.html#wp1008418
You mention that ports 80,69, and 443 are already open on the firewall, but CUE needs more than that in order to communicate with CUCM. CUE needs a TCP connection to CUCM on port 80, JTAPI connects to port 2748 (and 2749 for TLS) on CTIManager, and CUE will be exchanging RTP over UDP with phones on port numbers 20000 and above.
This is not an exahustive list, other ports may have to be opened up for things like email notifications, the Web interface, etc to work.
I hope this helps,
