It's not clear to me whether you want to clarify the term 'post mortem', or how to do a post mortem... so..
Post mortem basically means 'after death' - it's used to describe an analysis of a dead person, or in this context it means 'work out why something failed, but is now working again'.
Typically in IT this means analysing logs, or admitting that you aren't logging anything and engaging in some educated guesswork!
So in your examples:
Calls failing - you would need to analyse gateway logs or CCM logs that you have collected from the time of the failure.
WAN down - again, looking at logged info from routers, or you might rely on your service provider to assist
