On Step 11, the drop down doesn't have cisco as an option. Obviously, I am following the instructions on this tech note so any suggestions?

Make sure that you have enabled CTI application use for the user entered in Step 7 and associated a phone with this user in CCM admin.

Yes, as someone already mentioned make sure that CTI option is checked and phone is associated with that user (x) you r goin to use also check the CTI IP it should be the one where you are goin to start CTI service , if it doesnt work try to stop and restart CTI service...
