Does anyone remember the URL one has to type to see the hidden page to upload scripts in CRS 3.5.1 software that comes with callmanager 4.0(2a).

I have heard that you need to manually upload the ScriptUpload.class and TAPS.aef to the CRA.

If you are speaking of the CallManager Extended Services package, the page is not hidden but disabled. It is disabled because CRS is operating on a restricted license designed to host the IP/AA script and/or TAPS, and that's it. If you're supposed to have the small IPCC Express co-resident bundle that Cisco packaged for free with CallManager 4.0 for a while (maybe still going on?), then you should be able to upload scripts and you might have installed the wrong package of software.
If you are using CallManager Extended Services deliberately, and you are asking because you can't get the TAPS script to appear after running the TAPS installer, it is fixed in 4.0(2a)SR1. If you cannot upgrade, call TAC and reference the bug CSCef17749, and they will guide you through a workaround. If you are asking because you want to upload arbitrary scripts of your own, that would be a violation of the license and we would not be able to assist you with that. This is also why I am not publicly posting the workaround to CSCef17749.

If you have a fresh install of CM 4.0, do you have to install Extended services to run TAPS.
Or does the base install of CM 4.0 include it?

You still need to install Extended Services (or the full IPCC Express). The portion of TAPS that takes your call is still a script run by the CRA engine. There are some references to a standalone TAPS in the CCM 4 documentation, but this just refers to them having split the TAPS installer off into its own executable, as opposed to being part of the Batch Administration Tool install.
