Double Incoming Calls

I have a location that is reporting:

"Incoming calls sometimes pop up on the phone as two separate calls. When picking up the line, the call is disconnected and the 2nd call disappears a few seconds later as well. The calling party experiences that the call is simply disconnected."

It appears to be very random. Any ideas?

I've seen something very similar to this with a site that had analog FXO connections. The PSTN connection was punched incorrectly and the TIP of one line was double punched onto the RING of the other line. Both lines would light up on the gateway as incoming calls but shortly after going off hook, both calls would drop.
