anybody have any ideas?

I believe that you may be running into the bug: CSCdz79065

With CCM 3.3 this could be bug id CSCec25549 also, fixed in 3.3(3)es19 which 3.3(3)sr3 or higher should fix.
If this is 4.0x then could be bug id CSCeg39215 which is fixed in 4.0(2a) es 18 or higher. That is newer then 4.0(2a)SR1 so you will need to contact TAC to get that right now until a new SR comes out.
