@ms2435 hi, i am not sure if this is related to bug or HW failure. as a 1st step try to upgrade device to recommended version.
as of now below are the versions.

also capture the boot logs at boot start and share here if its possible.

Make sure it is inserted in the correct slot to work. Second, when you insert the card, what logs do you see on the console or cli?
Based on the error, I am thinking of other possibilities.
1. The card may be faulty
2. wrong slot
3. Insufficient power.
4. version of the code
