what I have on my CSV sheet  downloaded from Cisco 

 

Error I get from Cisco

 

(no way to attach a file to this .  So here it is typed out. )

Import Errors: Operations could not be complete.  {Unable to perform the operation = File Import failed. Error in CSV headers. } Retry
