 - Email/Cisco ID 

 - Company name:

 - Company Physical Address

 - Contract Associated

 - If need role to be assign on SNTC.

The customer was informed about that the user provided doesn't have roles assign on the portal and that he have to contact the delegated admin of the account and request the correct roles.
