4507 power supply problems

We seem to be seeing an awful high failure rate on the PWR-C45-4200ACV power supplies . Its in multiple different sites . We have probably replaced a half dozen in the last 6 months and these are less than 2 years old. What we see is this . They usually look like this . We have tried reseating the power supplies etc.. and nothing seems to get them out of the err-disabled state and the message that states they are different power supplies is false as both are exactly the same . Anyone seen this problem? If we get them RMA'd and a new one installed it looks fine.

We have experienced the same issue with the 4200W power supplies in India as well

Even though the Power supplies were same, the switch used to throw the mixed mode power supply problems..

However we have made one observation..The power supplies that are showing these errors are all manufactured in China. When we did our RMA recently we ordered for a different one and received a Sony Power supply which seems to be stable for the past 2 weeks
