We've seen this error a few times now on a 6500, SUP1A with different FastEthernet and GigE cards, and have replaced 3 different cards, in 3 different slots.
(As per the online documentation).
At this point we don't think the issue is card related (contrary to the documentation).
As of now we have switched to the standby sup module.
Any suggestions are appreciated

If the issue is not happening with the standby sup, then have you tried replacing the sup1a?
Have you tried the cards that failed in other working slots?
