Arp - Same mac in different ways

Hi,
My switch is generating logs stating the same mac in different ways.
Do you know why?
May 6 17:24:54 2016 Site1 %%10ARP/5/ARP_DUPLICATE_IPADDR_DETECT(l): Detected an IP address conflict. The device with MAC address 001b-4f4f-26ff connected to Bridge-Aggregation1 in VLAN 1 and the device with MAC address 001b-4f4f-26ff connected to Bridge-Aggregation2 in VLAN 1 are using the same IP address 192.168.123.229

whats the device connected for that mac is it wireless ?
whats the bridge aggregation1 and 2 , is the switch somehow leaning this through 2 locations on the switch to see this conflict ? whats the physical setup look like

Mark,
The mac (001b-4f4f-26ff) is an IP phone.
Bridge-Aggregation 1 have connected a switch and bridge-Aggregation other connected switch.
Yesterday I updated the switch software. Does this log is related to software version?
Since yesterday did not have this log.

It looked like the bridge aggregation didn't match between the 2 switches; so packets were being received "unexpectedly", which normally isn't a good thing. Glad to see the switches now appear to agree on channel operation (or have shut down the offending path).
