How did you try and open a TAC case, through the web or over the phone? I have always had to call Cisco and explain to them that I do not want to open a case based on a smartnet and that I just want to exercise the limited lifetime warranty. Also please be aware that the power supply is only under warranty for five years, so if the device is older then that you are probably SOL.

Post the first seven digits of your dead appliance's serial number.
LLW for the 2960 is valid if the appliance was manufactured after 01 May 2009.

Hmmmm ... Not covered.
Based on your output, the appliance was manufactured on March 2009. May not be covered.
