Do you have any file in flash:
can you post "sh flash"?

I found this:
Error Message
%IFS-3-FSDUP : Failed to add [chars], filesystem prefix exists
Explanation An internal software error occurred.
Recommended Action If the message recurs, copy the error message exactly as it appears on the console or in the system log, and report it to your Cisco technical support representative.
So your flash card might have an error. Try to post more information like Reza suggested.

Ian

That does not look good. Take the flash card out and try it in another router but I think the card itself is damaged/corrupt.
Try to format it with another application. Most card readers are able to take flash cards. But after formatting you will need to reload the IOS via tftp or xmodem. Here are the links:
http://www.cisco.com/en/US/products/hw/routers/ps259/products_tech_note09186a008015bfac.shtml
http://www.cisco.com/en/US/products/hw/routers/ps259/products_tech_note09186a008015bf9e.shtml
If after all that it doesn't work, card is probably corrupt and you will need a new card.

Sounds like the IOS is corrupt and has a bug. Can you eject the CF and put it into a multi-card reader. Copy a DIFFERENT IOS version into the CF and boot the new IOS?

Hi Abdulwali,
To recap, the IOS image on this CF is corrupted. Possibly the CF is corrupted.
First look at this Field Notice (FN):
http://www.cisco.com/en/US/ts/fn/620/fn62573.html
Your capture is not the classic result for this FN but there are some similar indications:
%Error formatting flash: (No device available)
If your CF matches the FN follow its instructions. If not follow this action plan:
The recommended action plan is as follows:
1. Take the CF to a PC and delete the old IOS image and copy a new IOS image onto this CF WITHOUT formatting it on the PC. Just do a copy.
2. Try to boot the 2811 with the new copy of the image.
3. If that does not boot and continues to give indications of a corrupted flash take the CF to the PC, format it, copy a clean IOS image onto it and try to boot the 2811 again. Formatting a CF for a router on a PC is a dicey thing and usually not recommended. But it is worth a try at this point.
4. If it still will not boot, use the instructions provided earlier to TFTP boot the system:
http://www.cisco.com/en/US/products/hw/routers/ps259/products_tech_note09186a008015bf9e.shtml
5. When it is running a clean IOS image, use IOS to format the CF. If this fails and you have a spare CF disk format it on this system.
6. If the original CF will not format but the new one will, open a TAC case to have the CF replaced by RMA.
7. If neither CF will format, open a TAC case to have the 2811 and the CF replaced by RMA.
This process should result in a clean system.
