Problem ( AVAYA / CISCO ) Voice VLAN

I have a device AVAYA MEDIA SERVER S8300 (DHCP Server for Voice VLAN)  that is connected to Cisco 3750X. The Cisco 3750X is also connected
to other 3 Cisco 3560X through trunk links . I'm implementing a Voice VLAN and Data VLAN . All services through Data VLAN
are working properly, but de Voice VLAN has a problem which is :

The Avaya IP Phones are signed correctly the first time, but sometimes disconnected and have to enter the phone manual programming once again for signing the right way on the AVAYA MEDIA SERVER.

Currently being used on some switch port to connect PC phones and other only voice service.

This is the configuration on 3750X :

interface GigabitEthernet1/0/19

description CONEXION CON AVAYA MEDIA SERVER S8300

switchport access vlan 200

switchport mode access

Trunk link :

interface GigabitEthernet1/2

description ENLACE TRUNK DE FIBRA CON IDF_C / IDF_C_SW1

switchport mode trunk

This is the configuration on 3560X:

Any Idea ???

Do you have your option 242 string set up in dhcp or are you using 46xxsettings.txt file? How are you putting these phones on vlan 200? Avaya won't pick up this vlan because they don't support cdp afaik...
