ROMMON Flash_Init not working

I have a Cisco 3650 that it booting into ROMMON.  In ROMMON, when I run dir I am only seeing these:

switch: dir

xmodem[0]: (read-only)
null[1]: (read-write)
bs[3]: (read-only)
tftp[12]: (read-write)
usbflash0[13]: (read-write)

 

So I run flash_init and it seems to run successfully:

switch: flash_init
Initializing Flash...

flashfs[7]: Bytes used: 1024

but if I run dir after the flash_init I still don't see flash:

switch: dir

List of filesystems currently registered:

usbflash0[13]: (read-write)

 

 

I have the bin image on the usb drive and tried booting to that image but get the following:

switch: boot usbflash0:cat3k_caa-universalk9.16.09.03a.SPA.bin
loading usbflash0:cat3k_caa-universalk9.16.09.03a.SPA.bin
Reading full image into memory...read: premature eof
usbflash0:cat3k_caa-universalk9.16.09.03a.SPA.bin: short read: bad mzip file

Error loading "usbflash0:cat3k_caa-universalk9.16.09.03a.SPA.bin"

Interrupt within 5 seconds to abort boot process.
Boot process failed...

 

Any ideas on how I can get this up?

Hello mcbrown,

try to boot using a TFTP server on your PC connected to the switch with a LAN cable.

follow the instructions on this document

Once you are able to boot with an IOS image you can try to format the flash: this might recover it or not.

Error message means the IOS file is corrupt. 

Go to the Cisco website and download the firmware again. 

IMPORTANT:  Make sure you compare the MD5 hash value of the file downloaded against the MD5 hash value found in the Cisco website.

Ok I downloaded the image again and tried booting from the USB and am getting this:
