That problem may reflect a hardware failure or it might be a software bug. In either case the best option would be to open a case with Cisco TAC.

Hello,

We have this issue too.

Anyone can confirm if this is a hardware or software issue?

Thanks,

Gee.  All it took was to read the first line of your attachment ...

Upgrade your switch to a more stable IOS version, like 12.2(55)SE9.  The IOS this appliance is currently running is one of the most buggiest IOS I've tested.
