Tacacs configuration but cant console and ssh

i configured Tacacs on my voice router gateway but can't perform ssh and console as expected.
not sure if my configuration missing.

when ssh - connection is refused

when console - it said %authentication failed and blank screen

yes you would need to put a :"login authentication company_tacacs+" statement on vty an console

because i couldnt access console, only option is to restart the device (i havent save it yet)

Apparently, the initial config already have , as below

line con 0
line aux 0
line vty 0 4
transport input ssh
! 

but seem like , when i ssh to the device , it would not accept the password of tacacs, 

is it because it is behind a ASA ?
before i config the tacacs config, i did ping the tacacs server, and it is replying
