WS-C2960X-48FPD-L Stuck in a loop

One C2960X switch from the switch stack is stuck in a loop

The switches were running c2960x-universalk9-mz.150-2.EX5.bin we was attempting to upgrade to c2960x-universalk9-mz.150-2.EX5.bin. 

All other switches upgraded to that version without an issue besides the one stuck in a loop currently

have you tried a factory defaults reset ?

Boot into rommon mode and either manually boot the old ios or delete the new ios, Once you have successfully done that then copy a new fresh ios on the switch and verify the it before booting to it.

1) Disconnect the switch from the stack
2) Attach console cable
3) Power up faulty switch and at the same time depressing the MODE button until STAT light goes out
4) Once in rommon mode type in sequence:

flash_init
load_helper
dir flash:  <-----look for old ios
boot flash:xxxxxxxOLD-IOS.bin

Pull the power cord. Don't use the "reload" command.
