so have you checked the obvious things, like can you ping it, is you 115 address responding to TFTP at all? is there any NAT in the path.
looks more like a connectivity related issue than anything else

Yes, you were correct.
There was Routing issue.
CUCM IP was not advertised in the VPRN/VRF of PE Router of the device.
