We have a EX90 with Multisite option installed.
if we upgrade or downgrade software what happen to that option.
we don't have option key.

upgrades/downgrades don't affect option keys, they will stay. Factory reset will remove them so don't do that.

Any upgrades or downgrades should not affect any of the installed Option Keys.
If you have managed to remove them, or they were removed by a factory reset of the endpoint, then you can obtain them again using the Cisco Licence Portal (https://www.cisco.com/go/license). Select "Get Licences" > "TelePresence Licence to Resend", then enter the Serial Number of your EX90 and click Search.
Any licence keys you have for your endpoint will then show on the screen along with the release key(s) to activate them on your endpoint.

Software version is TC7.0.2 that is deferred in cisco site.
Our EX90 don't have any contract , how can i reinstall or changing software?
( i don't have any .pkg file)
what is the solution?

If a software version is deferred, unless you have a saved copy of it when you initially downloaded it, there isn't a way to download it again. You have a few workarounds, if you have another EX or C Series endpoint that is attached to an active service contract and is associated to your account, you can download the software that way, as they all run the same software version. You can also contact TAC and request software through certain Critical or HIgh security advisories, for example cisco-sa-20170607-tele will get you access to TC7.3.8.

Our EX90 ... " Lost connection with control device".
I did factory reset but ... " Lost connection with control device".
i have only 5_1_13 pkg file
do you have any pkg file that i can install it ?

If your device doesn't have a "TC5" release key installed on it, then it still won't work even if you install TC5 on to it. (TC6.1+ doesn't require a release key).
If you need updated software, and don't have a service contract on your device, you can, as previously suggested by @Patrick Sparkman, contact the TAC and reference the cisco-sa-20170607-tele Cisco Security Advisory as per the "Customers Without Service Contracts" section of said advisory.
Customers who purchase directly from Cisco but do not hold a Cisco service contract and customers who make purchases through third-party vendors but are unsuccessful in obtaining fixed software through their point of sale should obtain upgrades by contacting the Cisco TAC:

Customers should have the product serial number available and be prepared to provide the URL of this advisory as evidence of entitlement to a free upgrade.
