Here is the scenario:
Both have 7940 Phones
President (x-201) is on a call line 1, Assistant (x-202). A second call comes in on x-201, The call should ring on line two of the assistant's phone. If not picked up, Call should route to voice mail box 201.
I cannot make the work.

So now you want it to ring on Mr. President's phone even though he's talking? My understanding was that you wanted it to ring on the assistant's phone if he was talking, and not to ring him at all. But maybe I misunderstood.
If you're wanting subsequent calls to ring in both places, the easiest way to get there from here is that you simply add 203 to Mr. President's phone as line #2.

Configure button 2 on the assistant phone as x203. Configure CFNA and CFB to forward to voicemail.
Configure President x201 to CFB to x203 (you could set no answer to go there as well, but you didn't ask for that). Set the busy trigger on this line to 1.
That should do it.

Sorry for the confusion. I will try to be more precise in the question. Can you make a suggestion on how I can make x203 ring simultaneously with the second call on x201 when the first call on x201 is in use?
the president would like to see the caller id and decide to pick it up or let it ring through to the assistant and if no one picks up it goes to x201 voicemail.
