My customer found error while login to attendant console with CUEAC 9.1 which integrate with CUCM10.5
After click ok, from console dashboard. It shows Not ready on status bar and incoming call to operator not forward to this console.
What is the cause for this error and how to check while issue occur?

A few things to check / try
It is a unique directory number and not a shared line
The device / phone is associated with the application user on cucm
Try deleting and recreating the operator related config and see if that helps.

I will check phone/device as your suggest. But deleting and recreating, which configuration that I should try (CUCM or CUEAC)
