I am not seeing installing IIS components in the install documentation but the Exchange 2003 system manager install has a dependancy. Anyone see this before and what did you do to correct?

Try Add or Remove Programs ---> Add/Remove Windows Components ---> Application Server ---> Internet Service Manager. This is for Windows 2003, whereas in Windows 2000, it would be dd or Remove Programs ---> Add/Remove Windows Components ---> Internet Service Manager
Also, if this is happening on Unity server, make sure that you only install the components required by Unity (Common Files,IIS, NNTP, WWW and SMTP)

Have not added the server to a domain yet. Is that a prerequisite for adding IIS?

No, it is not needed for IIS. IIS can work on a Windows Server in a workgroup also, but for Exchange to work, you will have to add this server to the domain as a member server
