After apply the configs to create a new FEX connections, I see the following FEX Status: Eth1/25 - Interface Up. State: Fabric Up

Try to use a different SFP or swap a known working SFP into that port.
Also try to use a different NIF on the FEX. (Network Uplink)
You could also try a reload of the FEX if you do not want to use another port. It seems that interface is not getting programmed correctly.
