Help Installing 12Gbps SAS 1GB FBWC Module for UCS C220 M4

Ordered a 12Gbps SAS FBWC Module for a new C220 M4 server.  I cannot locate any spec sheet or installation guide for this module.  The box included a small board, battery and some cables...

Anyone installed this before?
