This was helpful, and yes I was able to get the version updated with the .pkg file for now, will upload the cop to CUCM for further updates.
Note no step upgrade was needed in this case to update from 9.2...
Issue was related to NTP server issue.
Clock was not synced and reason why the disconnects were coming from Unity.
Once we updated a valid NTP server IP and synced the clocks, issue got resolved and no more disconnects from the IVR system (Unity)

Managed to sort out the rest of the devices:
Resolved by skipping 11.1.4.1 and upgraded to 11.5.2.4
I did notice like some sort of bug, image completes upload then shows "Installation failed" then if I click continue it just starts extracting cop fil...
