There are many solutions available using VoIP. Some of them are given below. As a end equipment you can use a ATA or a Analog phone or even a PBX.

You can use IP phone If you have Cisco call manager express in your router which depends on the IOS feature set running.

You can use the Cisco 827-4V ADSL Router at the branches, which comes with Voice interfaces.

QoS cannot be guaranteed over the internet. Just because your internet router may honor the QoS tag on a packet and place it into the appropriate queue for guaranteed delivery doesn't mean that your ISP's router will do the same. Or their router that connects them to the next tier provider or any router along the multiple hops from source to destination, for that matter.

Hope this helps
