Type of error that occurred last........................... AP got or has been disconnected
Reason for error that occurred last........................ Image data request received from an unsupported AP
Time at which the last join error occurred................. Jan 13 13:27:26.818

WLC version:Software Version  8.0.120.0

I would like to know if this is a IOS version issue or other fact caused this.

 - Is the AP-model supported by the WLC software version ?

AP model:AIR-LAP1262N-T-K9, site has 20 APs, they are in same model. so I would take a yes.

 - Appreciate the answer, but in this case  , I would let them extra verify this AP, to make sure that they didn't put a chicken between the ducks.

I would also upgrade to the latest 8.0 release, because yours has several known bugs and I think even some security issues.
See here for the release notes: https://www.cisco.com/c/en/us/support/wireless/5500-series-wireless-controllers/products-release-notes-list.html

thanks Patoberli,I would also intend to upgrade, but I guess I need a solid proof to share with my clients so this can be done.

thanks!

8.0.120 supports that AP model, does it occur on any other AP's of the same model? have you done a bug search?

thanks, Bernhard, I haven't done it yet, I guess bug serach only autherised by Cisco TAC?
