So from what I'm seeing in the debug, the client is getting authenticated just fine, what is failing is DHCP

 b8:5a:73:5e:f1:88 DHCP requested ip: 172.18.34.21

The client is requesting this address, but none of the DHCP servers ever reply to this request. You may want to check logs on your servers.

Make sure, is there any restriction enable for this MAC address B8:5A:73:5E:F1:88

at server side.

Totally Agree with Stephan. There is something wrong with DHCP.

Could you enable or Disable DHCP rpxy on your WLC and try again.

Controller > Advanced > DHCP
