Both power modules are in FAILED state. Please check the power

0/PS0/M0/SP     PWR-3KW-AC-V2             FAILED           PWR,NSHUT,MON

0/PS1/M0/SP     PWR-3KW-AC-V2             FAILED           PWR,NSHUT,MON

A bit confused, i've installed only one PSU on each Power Card. But IOS-XR displaying both PSU in failed state.
