Can anybody advise if it's hardware related?

Hi,
you gotta check:
admin sh env temperatures
admin sh env fan
also physical examination would be good - the air filters could got dust etc.

FAIL doesn't sound too good I must say, normally on a 12k with 4.x you should see FANs being rotating:
