CUACA has to be installed on a windows server.
Regarding SQL,
CUAC installation guide has all information which you are looking for. Read the guide corresponding to your version.
You can download the OVA from
Login>> Go to downloads >>
1.4 is the only available OVA.
Asper Virtualization for Cisco Unified Attendant Consoles (CUAC) guide, for CUAC 12 minimum required OVA is CUAC_v1.2_vmv10.ova.
You can use V1.4 template for your deployment.
I never used an OVA for CUAC. I always create a new virtual machine with specifications mentioned in the install guide. CUAC OVA is a new thing for me. I have a deployment in coming weeks, i will also try this OVA.

If you are looking for the VM OVA try this link:

How interesting that the download for 12.x is not in the Cisco Software... If I'm reading the documentation, you have to download it from Arc Solutions. They took over CUAC around version 7, but I thought Cisco had brought it back in-house. If you need the download software, I'd say contact your Cisco rep for assistance.
