Hi,
The "Invalid Entry(s), retry" error will be seen if any one of the login entries is not valid and also if the phone is not associated to the RM JTAPI Provider userid on the Call Manager.
If problem still persists, please open a TAC case and provide MIVR trace with debug levels SS_TEL, SS_RM, SS_CM, SS_RMCM & ICD_CTI which would provide more login info.
