i need the source app for UCCX cad for agent in hotline to sign in instead singing in through cisco  IP Phone direct

You get it from the UCCX server itself, from the AppAdmin web interface. It's under the Tools > Plugins menu option.

Keep in mind, that depending on your UCCX version and licensing level, you may not be entitled to using it though.
