If I have premium uccx license, would CAD and IPPA consume 2 seat licenses for same user/ext ?

No, CCX licenses are only consumed when agent is logged in, since agent cannot login to 2 different phones, you would use one license at a time.

Supervisors logged into CSD also checkout a license. Essentially each unique username logged in consumes a license.
