i have an issue with conference now CUCM 12.5
if i call from 7821 call immediately disconnected from other models it works ok (6921 for example

Conference Now Number must be reachable irrespective of the phone model. Check the Calling Search Space of the 7821 phone from which you are calling Conference Now Number.

When you calling conference now number are you getting the IVR ?
What you exactly mean by not working. Is it the IVR not playing or you are not able to login to your meeting number ?

Please check the phone configuration to ensure that it is enabled for Conference Now. If the phone is not configured correctly, Conference Now will not work. Additionally, as mentioned by @Nithin Eluvathingal, the phone's CSS must be reachable to the Conference Now partition.

Do you receive a fast busy tone or an error message?
Also please post the sdl trace to see if we find something there.
