I am using Jabber in phone mode only. Is the multi-line feature available for phone mode only or do I need a Jabber and Presence Server?

That’s a CUCM function so it should not require CUPS. All you need to have is a version of CUCM and Jabber that supports multi line.

I have installed Jabber 12-8-0 on my computer. I have Jabber phone mode only setup on CUCM, additional lines don't appear on the Jabber app nor does the app look like the one that is displayed on:

That version doesn’t have support for multi line in Jabber. If you read the post you referenced it says clearly that you need to be on 11.5 SU3 with a cop file, or you could be on SU4 and above without the need for the cop file.
